What is Aspire - Transition Towards Independence?
At Ignite Community Network, we are determined to support all individuals to live a full and active life. We understand that moving out of the family home is a key transition stage that everyone is confronted with at some point in their life. This could mean living independently in the community or through a formal accommodation program such as Supported Independent Living (SIL). The Aspire program will support you to understand what independent living looks like for you and the steps that you will need to undertake to enable you to move out of home.
The Aspire Program is designed to simulate an independent living environment over a 7-14 day period. As part of each stay we would replicate routines, activities, social programs or events that form a part of your life. In addition, the Asprire Program will help you to understand the responsibilities and opportunities associated with living out of the family home and the skills that you will need to sucessfully live independently.
The program will assist with developing the following skills:
- Meal preparation and cooking.
- Navigating the public transport system.
- Cleaning and domestic duties.
- Household maintenance.
- Vehicle maintenance.
- Organisational techniques.
- Washing and folding clothes.
- Grocery shopping and budgeting.
- Scheduling of social activities and events.
How will the Aspire Program support me?
The program will work with each individual and their family to develop an Individualised Transition Plan. This plan will become the key document that will be used to undertake the program.
The Individualised Transition Plan willl identify the following:
- Who you are, likes, dislikes, anxieties and key barriers.
- The roles that family, friends and community play in your life (Informal supports).
- Who you want to live with (alone or with a friend).
- Employment associated supports.
- Social activities in your life.
- Tasks that you complete independently or with minimal support.
- Tasks that you need additional support to undertake.
- Timeframe to transition towards independent living.
Program Structure and Eligibility
Aspire Structure:
The Aspire Program will be staffed 24 hours per day. Although the aim of the program is to simulate an independent living environment it is important that someone is available to provide support as required. This reduces the likelyhood of incidents occurring and promotes a safe place to practice new skills.
In addition, a mentor will be assigned four hours per day. The mentor will provide one-to-one support to undertake key activities, learn new skills and/or assist with daily routines.
The Aspire Mentor may assist with the following:
- Getting to and/or from work.
- Budgeting.
- Meal Planning
- Shopping.
- Organising social activities, for example, going to the movies.
Aspire objectives and key competencies are based on the Individual Transition Pogram that is developed at the commencement of the service. The timeframe for each competency is built into each transition plan and is reviewed at the completion of each stay.
At the completion of all competencies the individual will undertake a graduation. The graduation program will demonstrate the skills that the individual has learnt and will be awarded a graduation certificate.
Aspire Eligibility:
The Aspire Program is able to support anyone who does not require 24 hour supports that is typically funded through the Supported Independent Living Program. Should you be seeking SIL supports please speak with our friendly team at Ignite to discuss SIL supports further.
The Aspire Program supports individuals no matter where they are in their life. For example, whether you have lived in the family home for a number of years and feel you are ready to trial living independently, graduating from secondary school, or being discharged from hospital and require additional time to practice key independent living skills.
If you feel that you meet this criteria, then this is the program for you!
Accessing the Aspire Program?
In order to access the program you will need to undertake the following steps.
Step 1 – Screening to ensure that you are eligible to take part in the program.
Step 2 – Signing of Service Agreement: Prior to entering into a planning process you will need to sign a service agreement and enable funded supports to be booked.
Step 3 – Development of an Individual Transition Plan: Each individual and thier family (where applicable) will undertake a planning process. This will support to identify your goals, routines, barriers and the proposed timeframe to transition away from the family home.
Step 4 – Confirming the number of stays and dates: In order to take part in the program each individual will be required to stay for at least 7 days. Individuals can be booked in for four stays per year. Should you require additional stays please discuss this with your Support Coordinator, LAC or NDIS planner.
NDIS Funding:
In order to take part in the program you will need to have Short-Term Accommodation (STA) at a 1:2 ratio approved in your NDIS Plan. The duration of each stay will be a seven day period. Our friendly team will be able to provide a detailed quote as part of the service agreement process.

Our Services
Ignite Community Network promotes a person-centred approach to ensure that the person with a disability is in control of decision making in relation to goals, service delivery and the team that will deliver services. Ignite Community Network will establish a family network as a vehicle to promote a person-centred approach. Ignite Community Network will provide services within the Individual’s tenancy or where the person is sharing with another person. Support Workers will be employed in line with the Social, Community, Home Care and Disability Services Award and the Disability Support Workers Award.
Independent Living
Ignite Community Network promotes a person-centred approach to ensure that the person with a disability is in control of decision making in relation to goals, service delivery and the team that will deliver services. Ignite Community Network will establish a family network as a vehicle to promote a person-centred approach. Ignite Community Network will provide services within the Individual’s tenancy or where the person is sharing with another person.
Support Workers will be employed in line with the Social, Community, Home Care and Disability Services Award and the Disability Support Workers Award.
Mentor Support
- Support to access the community
- Support to facilitate/achieve health, wellbeing and fitness goals
- Support to participate in mainstream recreational activities
- Support to pursue hobbies and interests
- Support to build and maintain relationships
- Support to introduce the Individual to new social experiences
Skill Development
- Budgeting
- Obtaining a driver’s permit
- Cooking
- Domestic tasks
- Transport training
- Booking activities and events
Access to Employment
Ignite Community Network will support Individuals to develop skills to identify and participate in economic activities.
Strategies include:
- Vocational skills development
- Voluntary activities
- Employment support/coordination/ workplace orientation (time limited)
Transition & Support Planning
Ignite Community Network will facilitate services to respond to an unexpected event,crisis, life stage transition, interface with mainstream services as well as developing capacity and resilience strategies.
Activities that ICN will provide are:- Transition from secondary school (where a complex situation emerges)
- Support to maintain employment (where complex situations arise)
- Support to navigate mainstream services e.g. justice system, child protection
- Support to advocate and coordinate issues associated with tenancy
- Coordination of unexpected crisis situations
- Coordination of behaviour and medical supports
- Coordination/planning of changes to current care arrangements
Respite
Ignite Community Network will support the parent or guardian in their caring role. Activities that Ignite Community Network will provide include activities identified within the following Services:
- Mentor Support
- Daily Living
- Skill Development
- Access to employment
The function of the family network
Ignite Community Network promotes a person-centred approach to ensure that the person with a disability is in control of decision making of their goals, service delivery and the team that will deliver services. Ignite Community Network will establish a family network as a vehicle to promote a person-centred approach.
The family network brings together people within the family environment along with friends, health care professionals, community groups and other identified stakeholders to assist the Individual to set and achieve goals. The family network promotes open communication and provides safeguarding to assist the Individual to remain safe. The Individual is at the centre of the family network and has choice and control as to who is identified within the family network and the roles and responsibilities of network members.
The Family Network Coordinator facilitates the family network. The Family Network Coordinator will use the Individual’s Service Plan to develop the network. The Service Plan will define roles and responsibilities of each network member and will act as a agreement. The Service Plan will identify how support staff will be recruited, hours of work and activities to achieve goals. The family network will ensure that Individuals are recruited based on the needs of the Individual and their cultural and religious beliefs.
